The ESO User's Committee will meet on 25th and 26th April 2013 and would like information from you on your experiences using ESO facilities during Periods 89-91 and ALMA Cycles 0 and 1. We would also like your views on the special topic of this year "VLTI Operations", which you may find towards the end of the questionnaire. We would highly appreciate if you could spend a few minutes filling in this form. This information will be used by the UC to discuss with ESO members on ways to improve ESO facilities.

We ask for your e-mail address in order to be able to contact you in case we deem it necessary to get more details about your feedback. Your responses, however, are confidential and will not be forwarded in identifiable form to ESO.

The questionnaire has been tailored so that only questions that apply to your profile will show up in your screen. For questions where quantitative information is asked (e.g. percentages of successful proposals), approximate numbers are sufficient.
